MEMBERSHIP INQUIRY

By signing, I authorize Kawonu Golf Club and its affiliates to review my qualifications for membership, including my credit history, and release them from any related liability. I also authorize the entities listed in this statement to provide the requested information and hold them harmless.

All information provided will be kept confidential, except as required by law. I understand that membership is by invitation only and is granted at the club’s sole discretion, in accordance with its By-Laws, Rules, and Regulations.
